The Levie family has a rich history in the dental profession. It began with James Kellum Levie serving as a dental missionary in Korea for several years before returning to LaFayette in 1943. Later, his son Walter H. Levie graduated from Atlanta Southern Dental College in 1944 and began practicing in LaFayette in 1947 after fulfilling his military commitment. Walter’s practice was located above the Bank of LaFayette for many years before he relocated to the site where our office is today on East Villanow Street. Many years later, Walter’s son, Walter H. Levie Jr. (Chip) graduated from Emory School of Dentistry (formerly Atlanta Southern Dental College) and began practicing in LaFayette in 1981. Dr. Walter H. Levie Jr. still continues the rich dental history of his family by providing quality dental care to patients in his practice as well as participating in dental mission trips to locations including Cambodia and Mongolia.

Meet The Staff


Walter H. Levie Jr., DDS

A native of LaFayette, Dr. Chip Levie has been practicing general, cosmetic and family dentistry for over 33 years. A third generation dentist, Dr. Levie received his Bachelor of Science degree from Emory University and his Doctorate in Dental Surgery from the Emory University School of Dentistry.

Dr. Levie is dedicated to not only cosmetic transformations, but the overall health and well-being of every patient he sees. As a result, Dr. Levie pursues the most advanced continuing education opportunities available, in order to provide his patients with the best care using the most current techniques, procedures, and technologies. Dr. Levie is a member of Accelerated Dental Studies, which is the state’s oldest chapter of the Seattle Study Club network. He has also taken numerous courses at the prestigious Pankey Institute in Key Biscayne, Florida, to help his patients maintain their oral health with maximum comfort, function, and esthetics. This calculated approach allows Dr. Levie to treat the teeth, muscles of mastication (i.e., chewing muscles), and temporomandibular joint (“TMJ”) harmoniously for more stable and long-term results.

Dr. Levie is an active member of the First Baptist Church of LaFayette, where he serves on numerous committees. Over the years, Dr. Levie has taken part in mission trips both nationally and internationally. He currently lives on the farm where he reared his two grown sons, Trey and Ty. In his free time, Dr. Levie enjoys hunting, fishing, wood working, managing his farm, and spending time with Buttons, his Boxer.


Tyler K. Levie, DDS

Dr. Ty Levie joins our practice after recently graduating from the Indiana University School of Dentistry. Ty was born and raised in LaFayette, Georgia, where he grew up idolizing his father, Dr. Walter H. Levie, Jr., playing sports, and dreaming of one day becoming a dentist in his hometown.

Ty attended the Baylor School in Chattanooga, where he excelled in not only academics but also football, lettering three seasons and being named an all-state player in Tennessee his final two seasons. Upon graduation, Ty received a football scholarship to attend Samford University.

Ty graduated, cum laude, from Samford University with a Bachelor of Science in Biology. At Samford, Ty was inducted as a member of the Alpha Lambda Delta National Honor Society. Outside of the classroom, Ty excelled on the football field as a four-year varsity letterman. Ty was nominated as the September 2008 Athlete of the Month for the Southern Conference and was awarded the Gary Cooney Scholastic Award, which is presented to the Samford football player with the highest grade point average.

Ty’s other accolades include being named to the 2008/2009 ESPN The Magazine Academic All-District Teams, and a semi-finalist for the William V. Campbell Trophy. He was also nominated for the Allstate/American Football Coaches Association Good Works Team, which honors individuals who have made outstanding contributions in the areas of volunteerism and civic involvement.

While at Samford, Ty’s commitment to public service expanded beyond the classroom. Not only did Ty volunteer as an academic tutor for Samford student-athletes, he was also elected team leader of a Habitat for Humanity project. Ty traveled to the remote villages of Mongolia on a dental mission trip. Ty also volunteered at various non-profit organizations, including the Dr. William Roy Mercy Dental Clinic, in Chattanooga, where he recognized an increased need for dental access for many individuals. In preparation for dental school, Ty served as a microbiology research assistant at the University of Alabama at Birmingham School of Dentistry.

Ty received his Doctor of Dental Surgery from the Indiana University School of Dentistry. While in dental school, Ty volunteered for various organizations and regularly traveled within the greater Indianapolis area to provide dental services to low-income patients. During Ty’s fourth year, he was selected to serve as a teaching assistant to the first-year dental class with its pre-clinical hands-on courses. Ty was inducted as member of Omicron Kappa Upsilon, a dental honor society, which recognizes students with high scholarship, character, and potential for leadership, and who are in the top 12% of the graduating class.

Ty’s professional affiliations include the American Dental Association and the Indiana Dental Association.

Aside from practicing dentistry, Ty enjoys remaining active, working out, hunting, and fishing. He also loves spending time with friends and family.


Stephanie Cooper, RDH - Office Manager

Stephanie has enjoyed over 10 years in the dental field as a hygienist and office manager. She graduated in 2005 from Chattanooga State Technical Community College with a degree in Dental Hygiene. She worked as a hygienist for four years before joining Levie Family Dentistry in 2009.

Stephanie is married to Matt Cooper and has two young children, Luke and Lauren. Her greatest joy is to spend time with her family and friends. She resides in Chickamauga, Georgia and is an active member of Oakwood Baptist Church.


April Kelley, RDH - Dental Hygienist

April joined the team at Levie Family Dentistry in 2008. She is a resident of Walker County and graduated from LaFayette High School. She graduated from Chattanooga State Technical Community College’s Dental Hygiene program in 2014. While in the dental hygiene program, April served as president of her class and received the Hinman Scholar Award for academic excellence.

Outside of the office, April enjoys spending time with friends and family. Her hobbies include crafting and cooking. She is passionate about improving the dental health of her patients through educating them on the benefits of oral health.


Michelle Crowder, RDH - Dental Hygienist

Michelle has been a life-long resident of LaFayette. She began her career in the dental field in 2005 as a dental assistant. She graduated from Chattanooga State Technical Community College’s Dental Hygiene program in 2009 and received the Golden Scaler award for the most outstanding clinician in her class. She joined our team in May 2013.

She enjoys volunteering and giving back to her community. She is a very outgoing, energetic young lady who thrives to educate patients on their need to better their overall health. She takes joy in spending time with her family and friends.


Haley Kokoszka, RDA - Dental Assistant

Haley has been a resident of Walker County for over 20 years. She is a CDA/RDA graduate. Haley earned her Dental Assistant degree from Chattanooga State Technical Community College in 2014, where she served as Vice President of her class. Haley joined Levie Family Dentistry in 2014.

Haley enjoys coming to work every day and loves how friendly and close her work family is. Haley played softball throughout college and continues to live a healthy and active lifestyle. She is very close with her family and friends and loves spending time with them.
